Before arriving for your first appointment, please fill out an admission form. I can send it to you by mail or FAX, or you can download a PDF admission form to print yourself. If you don't have the necessary Adobe Reader software, you can download it free by clicking on this icon:
Insurance:
I accept most major insurance providers and will happily bill your insurer for your visit. It would be helpful for you to call your provider in advance to determine what your coverage is. You can contact me to send you a checklist or download a PDF list of questions to ask and a form to fill out. This will facilitate your first appointment.
